RED BANK FORMS & ADDITIONAL DOCUMENTS

Welcome to the 2017 Red Bank Youth Football and Cheer Season. Registration will be held on Feb 25th, March 25th, April 29th, & May 20th from 11 a – 1p at Count Basie Field. There will be board members and coaches available to answer any questions you may have regarding schedule, equipment, costs, philosophy and play policy.

Prior to the registration date, please visit our website to download and print the required forms. Please complete all forms and bring them with you to registration to ensure a quick registration experience.

REGISTRATION CHECKLIST
If its easier of you to register at one of our in person registrations, the following serves as a checklist to ensure you have everything you will need! Please Print All of the Forms Below, complete, and bring with you.

  • 2017 Red Bank Registration Form
  • 2017 AYF AYC Player Track Form   – 2 Pages – make sure you initial (5 times) and sign all lines on Page 2
  • Current wallet size photo of your child
  • Copy of Athlete’s Birth CertificateNew players only
  • 2017 Emergency Medical Form – 1 Page – Signature at bottom required
  • 2017 Medical Clearance Form – 1 Page – Must be signed by a doctor and dated after January 1, 2017. This form must be handed in BEFORE the first practice or your child will not be allowed to participate.
  • 2017 Red Bank Consent Forms – Parent Code of Conduct, Photo/Video Release, Equipment Deposit & Work Bond, and Refund Policy – Please sign and date each section
  • Registration Fee (Cash or Checks Made out to Red Bank AYF)
